Written Request Form

Instructions: This form helps you to submit a formal written request to The Office of the Privacy Commissioner for Bermuda (“PrivCom”), if you have concerns about how an organisation is using or has used personal information in accordance with the Personal Information Protection Act 2016 (“PIPA”).

Privacy Notice: A copy of this form will be shared with the organisation involved in your written request. PrivCom may redact any information contained in the written request that we feel may be appropriate, prior to giving a copy of the written request to the organisation or any other individual affected by the written request.

I believe the organisation has withheld and or redacted some of my personal information
The organisation has refused my written request due to relying on an exemption or exclusion
